Posterior Tibial Tendon Dysfunction: A Common Cause of Adult Flatfoot

Everyone knows what a flatfoot looks like, but far fewer people connect it to posterior tibial tendon dysfunction. The tendon that runs along the inside of the ankle holds the arch in place, and once it starts to break down, the whole foot pays for it. Most people have never even heard the term PTTD until their own foot starts acting up.

The pain usually shows up on the inside of the ankle first, then spreads to the arch as the foot begins to flatten. Why does flatfoot hurt so much by the end of a long day? Because the tendon is doing extra work just to keep the arch from collapsing further, and that strain adds up fast.

This guide walks through the symptoms, the causes, how the condition progresses, and what actually helps. By the end, you'll have a clear picture of what PTTD looks like and what your options are if you're already dealing with it.

What Are the Symptoms of Posterior Tibial Tendon Dysfunction

The earliest sign is usually pain and swelling along the inside of the ankle, right where the tendon runs. Walking, climbing stairs, or standing for a while tends to make it worse, and the area often feels warm to the touch.

As the condition progresses, the arch starts to flatten, and the foot can begin to roll inward. Some people notice their shoes wearing unevenly before they even feel much pain, which is a small clue worth paying attention to.

Common symptoms include:

  • Pain and swelling on the inside of the ankle
  • A visible flattening of the arch over time
  • Difficulty standing on the toes of the affected foot
  • Pain that shifts to the outside of the ankle in later stages
  • Fatigue in the foot or leg after standing for long periods

These symptoms rarely show up all at once. They tend to build gradually, which is exactly why PTTD gets missed or brushed off as ordinary foot soreness for so long.

What Causes Posterior Tibial Tendonitis

Overuse is the biggest driver behind posterior tibial tendonitis. Runners, hikers, and anyone on their feet for work put repeated stress on the tendon, and over months or years that stress wears down the tissue faster than the body can repair it.

Age plays a role too. The tendon naturally loses some of its blood supply and elasticity as people get older, which makes it more prone to injury even without an obvious cause. Women over 40 tend to be affected more often than men.

PTTD is one of several foot and ankle conditions that develop slowly and get worse without treatment. Obesity, diabetes, and hypertension can all speed up the process, since they affect blood flow and tissue health throughout the body.

Injury can also trigger it directly. A hard fall, a bad twist, or repeated small strains from certain sports can damage the tendon suddenly instead of gradually, and that damage often shows up as pain within days rather than months.

Stages of Posterior Tibial Tendon Dysfunction

Doctors generally break PTTD into four stages, and where someone falls on that scale determines what treatment looks like. Getting an accurate stage typically means a physical exam paired with imaging like X-rays or an MRI to check how much the tendon has degraded and whether the joints have started to shift. Catching the problem early, before those changes set in, usually makes for a simpler and more effective treatment path.

Stage one involves inflammation without much visible deformity. The tendon is irritated, but the arch still holds its shape.

By stage two, the arch has started to flatten, and the foot may shift outward when viewed from behind. Walking becomes noticeably harder, and standing on the toes of the affected foot gets difficult or even impossible.

Stage three brings a rigid, fixed flatfoot. The joints have adapted to the new shape of the foot, and simple stretching or bracing won't reverse that on its own anymore. Pain often spreads to the outer ankle as the heel bone shifts position.

Stage four is the most advanced, in which the ankle joint itself becomes involved and begins to tilt. At this point, the damage extends beyond the tendon and into the surrounding joints, which usually calls for more involved treatment.

How to Treat Posterior Tibial Tendon Dysfunction

Treatment depends heavily on which stage someone is in. Early cases often respond well to rest, anti-inflammatory medication, a short course of oral steroids, and a supportive walking boot, brace or orthotic that takes pressure off the tendon while it heals.

Physical therapy plays a big part in recovery for most people. Posterior tibial tendonitis exercises can strengthen the muscles around the ankle and slow the progression of the condition, especially when they're started before the arch has flattened too far.  Physical therapy modalities can prove to be very instrumental in decreasing inflammation.

Custom orthotics or ankle braces work well for stage one and two cases, giving the arch mechanical support it can no longer provide on its own. Some people wear these long-term, while others use them only during flare-ups.

When conservative options stop working, PTTD surgery becomes the next step. Surgical approaches range from tendon repair to more involved reconstruction of the foot's structure, and even foot fusion in severe cases. Recovery after PTTD surgery can take several months, so doctors usually reserve it for stage two and beyond, once bracing and therapy haven't held the line.

Is It Okay to Walk With Posterior Tibial Tendonitis?

Light walking is usually fine in the early stages, as long as it doesn't cause a spike in pain or swelling afterwards. Pushing through sharp discomfort, though, tends to make the underlying damage worse rather than helping it heal.

Supportive footwear makes a real difference here. Shoes with good arch support and a stable heel reduce the strain on the tendon with every step, which matters more than most people expect once the condition sets in.

Is PTTD a disability in a legal or medical sense? It can be, depending on severity and how much it limits daily function, though most cases don't reach that point if they're caught and treated early. Exercises for PTTD, combined with proper footwear, keep many people mobile without ever needing that kind of classification.

Anyone unsure whether their pain level is safe to walk through should check with a podiatrist rather than guessing. A quick evaluation can settle the question and prevent months of unnecessary strain on a tendon that's already struggling.

Posterior tibial tendon dysfunction doesn't announce itself loudly, but it rarely gets better on its own either. Catching it early, while the tendon is inflamed rather than fully collapsed, gives someone the best shot at avoiding surgery and keeping their natural arch intact.

Whether that means a brace, a round of physical therapy, or a conversation with a specialist about next steps, the earlier the response, the more options stay on the table. A little attention now saves a lot of trouble later.
